Microsurgical perforator flaps, which do not sacrifice
muscle to reconstruct the breast, are being used more frequently. Deep Inferior Epigastric Perforator (DIEP) and Superficial
Inferior Epigastric Artery (SIEA) flaps recreate new breasts with your own abdominal skin and fat, while the Superficial Gluteal
Artery Perforator (SGAP) and Inferior Gluteal Artery Perforator (IGAP) flaps use skin and fat from the buttocks. The Transverse
Upper Gracilis (TUG) flap is another less frequent option using tissue from the inner thigh (including the muscle, although
this doesn't impair strength or motion). Flaps of skin and fat can also be moved from hips ("love handles").
If you don't have enough fatty tissue to reconstruct the size breast you want, some surgeon will use flaps from two donor
sites, stacking them together to provide more fullness and volume. Although fewer surgeons are qualified to perform these technically-demanding, exacting surgeries,
our list is constantly expanding. If you're considering reconstruction with a perforator flap, consider the following
list of surgeons who perform this procedure. Their offices will help you with insurance issues and travel arrangements. Many
women consider the result well worth the travel to one of these surgeons. (Please contact us if you know of a surgeon who
performs any of these procedures but is not listed below.)
A surgeon's experience is of the utmost
importance. Some surgeons on trained on perforator flap techniques but actually perform the surgeries infrequently.
If you're considering a perforator flap reconstruction, look for a surgeon who is very experienced in the type of flap
you want. Be sure to ask how many of these procedures he/she has performed and what his/her failure rate is.
